Kovid Death Certificate: How to apply for compensation and what to do?

Health Minister Veena George has said that applications for Covid 19 death appeal and certificate can be filed in the state from October 10. Although the Government of Kerala issues the Covid Death Certificate, the new system is based on the revised guidelines of the Union Ministry of Health and the ICMR as directed by the Supreme Court.

According to the revised directive issued by the ICMR, any complaint which may be declared as Covid death and not yet on the Covid death list declared as Covid death by the Government of Kerala, can be appealed in a transparent manner through the new system.

It is arranged in such a way that things can be done relatively easily without any difficulties. You can apply online or in person. For those who do not know how to apply online, PHC You can apply online by submitting the required documents through the Akshaya Center. Official Covid death certificate will be issued after detailed examination of the applications received. The decision on the application is made online. Applications received will be processed within 30 days.

The first thing to do

The application for death diagnosis and certificate should be made through the e-Health Covid 19 Death Info portal. First go to Covid 19 Death Info Portal (https://covid19.kerala.gov.in/deathinfo) and make sure your name is on the list of those killed by Covid. It is sufficient to apply only if there are non-participants.

How to apply?

First go to https://covid19.kerala.gov.in/deathinfo and click on Appeal Request. Then type the mobile number in the page that appears and type OTP. Click for number. OTP available on mobile Enter the number and click on Verify. You will need to enter the correct information on the next page. Type the death registration key number of the local body and upload a copy of the death certificate. The key number appears first on the top left of the death certificate.

Name of death certificate received from the local body, age, gender, name of the father or mother or husband, mobile number in the hospital records, address on the death certificate of the local body, district, name of the local body, date of death, place of death, district where the death was reported, local body which issued the death certificate Name and hospital where the death was confirmed should be given. A copy of the relevant hospital records should also be uploaded. Finally the details of the applicant should also be given.

The information provided by the applicant should be re-examined and submitted. After successful submission of the application, the application number will be sent to the applicant’s mobile number. The successfully submitted application is sent to the hospital where the death is confirmed for processing and then to the District Covid Mortality Committee (CDAC) for approval. According to the new ICMR guidelines, the new certificate will be issued after approval by the District Covid Death Assessment Committee (CDAC).

To know the status of the application submitted

Click on the Appeal Request and check your request status to know the status of the application. The date of death and the application number or the mobile number of the applicant given earlier must be provided. The status of the application can be ascertained by providing the correct information.

ICMR. How to apply for the certificate in the sample?

Go to the link https://covid19.kerala.gov.in/deathinfo. ICMR Click on Certificate Request. As before, the mobile number is also OTP. The number must also be entered. Type the death registration key number of the local body and upload a copy of the death certificate. In addition, the death declaration document number obtained earlier from the Department of Health and a copy of the certificate must be provided. The name of the local body which issued the certificate, the name of the death certificate obtained from the local body, the name of the father or mother or husband, age, date of death, the district where the death was reported, the name of the local body which issued the death certificate and the details of the applicant should be given. You can make the necessary corrections and submit. The application number will be available on the mobile number of those who have successfully submitted. It will be sent to the District Covid Death Determination Committee (CDAC) for approval and a new certificate will be issued as per the ICMR guidelines.
